About Us
The Central & Western DHC Express provides district-based primary healthcare services through health promotion, diseases prevention, health assessment & consultation and chronic diseases management (e.g. hypertension, diabetes and musculoskeletal disorders etc.) to enhance public awareness of self-management of health. The Central and Western DHC Express helps strengthen the involvement of individuals and communities, plans and coordinates among various medical and social sectors and aims to encourage the public to take precautionary measures against chronic diseases, enhance their capability in self‑care and home care, and reduce the demand for hospitalisation.
The Food and Health Bureau (FHB) awarded the 3-year operation service contract of Central & Western DHC Express to The Hong Kong Society for Rehabilitation on 26 April 2021.
The Hong Kong Society for Rehabilitation aspires to be an effective, impacful and caring organisation in holistic care and rehabilitation.Through innovation in rehabilitation and empowering persons with disabilities or health conditions, we advocate holistic well-being, social participation, and an inclusive and enabling environment.
Our Services
The Central & Western DHC Express provides three levels of disease prevention services. Residents or working individuals of Central and Western District can register with their proof of address/employment and identification as members and participate in the three-tier prevention project:
